Discharge Coordinator

The Discharge Coordinator is responsible for managing and streamlining the discharge planning process for clients completing treatment at Eagle Lake Recovery Center. This role ensures clients transition out of residential care with safe, supportive, and clinically appropriate aftercare plans. The Discharge Coordinator works closely with clients, families, and the clinical team to coordinate referrals, housing, transportation, and continuity of care.

Key Responsibilities

Discharge Planning

  • Collaborate with the treatment team to begin discharge planning at admission or early in care
  • Create individualized aftercare plans that include outpatient treatment, psychiatric follow-up,     recovery meetings, and support systems
  • Schedule appointments with therapists, IOP/PHP programs, MAT providers, or other services
  • Assist clients with applying for benefits, housing, or transportation if needed

Client& Family Support

  • Meet with clients to review discharge options and build relapse prevention plans
  • Provide psycho education on recovery resources, self-care, and crisis planning
  • Coordinate family meetings (as needed) to support safe reintegration
  • Ensure clients understand and sign all discharge documents and referrals

ReferralCoordination

  • Maintain a network of trusted outpatient providers, therapists, sober living programs, and support services
  • Communicate with external agencies to schedule and confirm referrals
  • Follow up to ensure warm handoffs and documentation of continuity of care

Documentation& Compliance

  • Complete and upload all discharge paperwork to EMR within 72 hours of discharge
  • Track discharge dates, outcomes, and referrals for internal reporting
  • Assist in outcome surveys or alumni check-ins if applicable
  • Ensure discharges meet all clinical and regulatory standards

Qualitications

  • Bachelor’s degree in Psychology, Social Work, Human Services, or related field (preferred)
  • 1+ years of experience in case management, behavioral health, or substance use treatment
  • Strong understanding of community resources, levels of care, and relapse prevention
  • Excellent organizational and communication skills
  • Proficient in EMR systems and Microsoft Office / Google Workspace
  • Ability to work with clients from diverse backgrounds in a trauma-informed and nonjudgmental way
  • Minimum of one year sobriety if in recovery

Schedule and Environment

  • Full-time, on-site position
  • Typically Monday–Friday with some flexibility for evening discharge planning
  • Works in close collaboration with clinical, medical, and case management teams
